Membership - Direct Primary Care
NOTE: We currently do not accept patients under 16 years old & do not provide narcotic pain management treatments.

Membership includes most primary care services. It also includes discounts on labs, imaging, prescriptions, supplements and integrative wellness therapies.​ Please note Membership - Direct Primary care have a 6 months minimum membership required.

Due at your first visit
ONE-TIME ENROLLMENT FEE: $425.00

A $125.00 non-refundable deposit is required to secure your visit and will be applied to your balance.

A credit card is required on file for monthly auto-charge and other services. Payment is due at time of service. You will not be charged until you are accepted as a patient.

A 6 month minimum membership is required before cancellation.

Membership is charged once a month as an auto-charge, based on the monthly membership pricing listed below.
